In 2026, we were approved as a subcontractor facilitator to Public Partnerships LLC (PPL), New York State's statewide fiscal intermediary for the Consumer Directed Personal Assistance Program (CDPAP).

What Is CDPAP?

The Consumer Directed Personal Assistance Program gives eligible Medicaid recipients the power to choose, hire, train, and supervise their own personal assistants — including trusted family members and friends.

You Are in Control

Unlike traditional home care, CDPAP puts you in the employer's seat. You choose who provides your care — a family member, a trusted friend, or anyone you feel comfortable with. You set the schedule, train your assistant, and direct your own care plan.

Hire Someone You Trust

Many consumers choose to hire an adult child, sibling, or close friend as their personal assistant. Your PA does not need to be a certified home health aide — they just need to be someone capable of providing the care you need.

Medicaid Funded

CDPAP is available to eligible Medicaid recipients in New York State who have a stable medical condition and require assistance with activities of daily living or skilled nursing tasks. There is no cost to you for facilitation services.

Stay in Your Home

CDPAP helps people with disabilities and chronic conditions remain in the comfort and familiarity of their own home — rather than moving to a nursing facility or other institutional setting. Independence matters, and CDPAP makes it possible.

Flexible Scheduling

You set the hours that work for your life and your needs. Whether you need a few hours a day or round-the-clock support, CDPAP can accommodate a wide range of care schedules as authorized by your Medicaid Managed Long-Term Care (MLTC) plan.

Wide Range of Tasks

Personal assistants under CDPAP can help with activities of daily living such as bathing, dressing, grooming, meal preparation, mobility, and toileting. They can also perform certain health-related tasks that would otherwise require a licensed professional.

Enroll with PPL

We help you complete enrollment with Public Partnerships LLC (PPL), New York's statewide fiscal intermediary. We handle the paperwork and coordinate with your MLTC plan or Local DSS.

Hire Your PA

Choose your personal assistant — a family member, friend, or other trusted individual. We guide you through onboarding your PA with PPL, including background checks and required documentation.

Ongoing Support

We provide continued support with timekeeping, scheduling questions, plan changes, and any issues that arise — for as long as you participate in CDPAP. We're a phone call away.

Who May Qualify for CDPAP?

To be eligible for CDPAP in New York State, consumers generally must meet the following criteria:

  • Enrolled in New York State Medicaid
  • Have a medically stable chronic illness or disability
  • Require help with activities of daily living or skilled nursing tasks
  • Be capable of self-direction (or have a designated representative who can)
  • Be enrolled in or eligible for a Medicaid Managed Long-Term Care (MLTC) plan or have Local DSS authorization
  • Reside in a private home or community setting (not a nursing facility)

Working with Public Partnerships LLC (PPL)

In 2025, New York State consolidated CDPAP fiscal intermediary services under Public Partnerships LLC (PPL) as the single statewide fiscal intermediary. This transition was designed to bring consistency, efficiency, and accountability to CDPAP statewide.

Accredited Home Care Services is an approved subcontractor facilitator to PPL. This means we work hand-in-hand with PPL to guide consumers through enrollment, assist personal assistants with onboarding, and provide ongoing support throughout your participation in the program.

What PPL Handles

PPL, as the fiscal intermediary, is responsible for Medicaid billing, payroll processing, and administering benefits for personal assistants. You don't have to worry about writing checks or handling taxes — PPL takes care of it all so you can focus on your care.

Roles & Responsibilities

You (Consumer) Hire, train, schedule, and direct your personal assistant. Make decisions about your own care needs.
Accredited HCS (Facilitator) Assist with enrollment, onboard your PA with PPL, provide ongoing support with timekeeping and program questions.
PPL (Fiscal Intermediary) Process Medicaid billing, manage payroll and taxes for your PA, administer PA benefits and worker's comp.
MLTC / Local DSS Authorize your hours of care based on assessment of your medical and functional needs.
Your Personal Assistant (PA) Provide hands-on care, record hours in PPL's timekeeping system, follow your direction and care plan.

Our Facilitator Services

Every service we provide is free of charge to eligible CDPAP consumers. We're here to make the program work for you.

CDPAP Enrollment Assistance

We help new consumers understand program requirements, gather necessary documentation, and complete enrollment with PPL and your MLTC plan or Local DSS office.

Personal Assistant Onboarding

We guide your chosen personal assistant through the PPL onboarding process — including background checks, required paperwork, training on the timekeeping system, and benefit enrollment.

Timekeeping System Support

PPL uses an electronic visit verification (EVV) system to record your PA's hours. We train consumers and PAs on how to use it correctly, and troubleshoot any issues that arise.

Ongoing Consumer Support

Our coordinators are available by phone, video call, or in-person appointment. We help with plan changes, adding or replacing a PA, addressing service issues, and connecting you to additional community resources.

Multilingual Phone Support

Our phone team can assist you directly in English, Mandarin, Cantonese, Spanish, Hindi, Urdu, Bengali, and Russian — no third-party interpreter needed. In-person offices also offer Albanian and Haitian Creole. Language should never be a barrier to accessing care.

Rights & Advocacy Information

As a not-for-profit with deep roots in independent living, we inform consumers of their rights under CDPAP, help them navigate disputes, and connect them to advocacy resources when needed.
